Print this page and take it to the class each time it is taught.
Write date and time of class, subject matter taught and names of students present.
Indicate how many hours are left that the student has purchased.
Student will write his/her initials to show that he/she attended class.
Please be sure that this sheet is always up to date. It will be used at the end of every month for your payment.
For offsite classes: every weekend send us email telling us how many hours you have taught,
and the names of the students.
Instructor and student will attempt to give at least 24 hour notice of cancellation or postponement.
If instructor gives less than 24 hour notice student has the option to NOT pay for the class.
If student gives less than 24 hour notice, student is charged for the class
